An emergency presentation of COPD provides an opportunity to review your patient’s medication and management and to offer education so that the risk of future hospitalisations can be reduced through better disease control.

The 2019 GOLD strategy states that, “Inhaler technique needs to be assessed regularly” and there is increased evidence for self-management and the use of pulmonary rehabilitation for COPD patients.1

This discharge pack includes a useful checklist for you to complete with your patient when they are discharged from hospital, back into primary care, with copies for them and their GP so that everyone has the same information. It covers a review of their smoking status, a reminder to check inhaler technique and to offer appropriate retraining where relevant, and a discussion of pulmonary rehabilitation and self-management plans. Also included are an oxygen alert card (for relevant patients) and a form for their GP, to be completed if their medication has changed. All this is presented in an informative eye-catching folder.
