What is connected care?
In recent years, technology has been harnessed as a potential solution to the challenges that exist within modern healthcare systems.
Connected care involves the integration of health technology, mobile devices and digital communication in order to connect patients, caregivers and healthcare professionals (HCPs) through universal access to data and relevant information.1 We at Teva believe that this connectivity has the potential to enhance the quality and continuity of care delivered to patients, maximising healthcare resources and ultimately improving health outcomes.
Sharing responsibility, saving costs and improving care
According to the World Health Organization, chronic health conditions are the leading cause of global mortality.2
With the growing prevalence of chronic disease, and the associated economic, financial and health burdens, the need for integrated and connected healthcare is becoming ever more pressing.
Monitoring health through digital technology encourages people with chronic conditions to be more involved in the management of their disease.1,8 Effective self-management enables health issues to be addressed before they become acute; this not only improves health outcomes, but it reduces the likelihood of hospitalisations and the corresponding strain on healthcare resources.1
Connected healthcare also carries the potential for HCPs to deliver personalised care. Digital health technology can reveal insights into treatment effectiveness on an individual level that can make treatment decisions more informed; by understanding how an individual lives with their condition, HCPs can tailor their care to the unique needs of each patient.
A study discovered that data from Fitbit®—a wearable activity tracker— can be useful in monitoring the prognosis of men with prostate cancer, whose treatment side effects can be mitigated with physical activity.9
